What should I bring to my appointment?

Beyond the completed forms, you should also bring identification, insurance cards, a list of medications, and any other information that may help us assist you on your periodontal journey.

If I make a mistake on my forms, what should I do?

If you choose to print the forms and bring them with you to your dental appointment, you can simply print new ones or cross out the incorrect information. However, if you submit your forms via the online portal and notice a gap in your information, please call us directly at (248) 327-1919. We’ll happily change anything that needs revision.
